Sometimes, the most difficult decision is to attempt limb salvage
خلال الفترة الماضية، تعاملت مع حالتين من أكثر الحالات تحديًا في جراحة العظام:
🔹 الحالة الأولى:
ورم عظمي خبيث في عظم الفخذ (Osteosarcoma)، استدعى استئصالًا واسعًا أدى إلى فقد عظمي يقارب 20 سم.
🔹 الحالة الثانية:
فقد عظمي شديد في عظم الساق (≈18.5 سم) مع التهاب مزمن، حيث كان خيار البتر مطروحًا بشكل جدي.
في كلا الحالتين، لم يكن التحدي تقنيًا فقط، بل كان قرارًا طبيًا وإنسانيًا معقدًا:
هل نختار الحل الأسرع (البتر)، أم نخوض طريقًا طويلًا ومعقدًا في محاولة إنقاذ الطرف؟
تم اعتماد استراتيجيات علاجية متقدمة متعددة المراحل، شملت:
- تقنيات Masquelet
- النقل العظمي التدريجي (Distraction osteogenesis)
- الدمج بين التثبيت الخارجي والداخلي
- إعادة البناء البيولوجي للعظم
النتائج:
✔ إعادة بناء عظمية ناجحة في الحالتين
✔ السيطرة على الإنتان في الحالة الأولى
✔ عدم وجود نكس ورمي خلال 4 سنوات في الحالة الثانية
✔ استعادة القدرة على المشي وتحمل الوزن
لكن الأهم من النتائج الجراحية، كان الجانب الإنساني…
حيث عبّر أحد المرضى بعد شفائه بقوله:
“لم تكن هذه مجرد عملية جراحية، بل رحلة أعادت لي الأمل قبل أن تعيد لي القدرة على الحركة…”
هذه التجارب تؤكد أن:
الطب ليس فقط قرارات سريعة، بل التزام طويل، ورؤية، وإيمان بإمكانية الشفاء حتى في أصعب الظروف.إنقاذ الطرف ليس دائمًا الخيار الأسهل… لكنه في بعض الحالات، الخيار الذي يستحق المحاولة.

Not every complex case ends with amputation.
Sometimes, the most difficult decision is to attempt limb salvage.
Recently, I managed two highly challenging orthopedic cases:
🔹 Case 1:
A massive infected tibial bone defect (~18.5 cm), where amputation was strongly considered.
🔹 Case 2:
A malignant femoral tumor (osteosarcoma) requiring wide resection, resulting in a bone defect of nearly 20 cm.
In both cases, the challenge was not only technical, but also a critical medical and human decision:
choose the faster solution (amputation), or pursue a long and complex limb salvage strategy.
A staged multidisciplinary approach was applied, combining:
Masquelet technique
Distraction osteogenesis
External and internal fixation
Biological bone reconstruction
Outcomes:
✔ Successful bone reconstruction in both cases
✔ Infection control in the first case
✔ No tumor recurrence at 4-year follow-up in the second case
✔ Independent ambulation
Beyond surgical success, the most meaningful impact was human. One patient wrote:
“This was not just a surgical procedure, but a journey that gave me back hope before giving me back my limb…”
These experiences reinforce that:
Medicine is not only about quick decisions, but about persistence, vision, and believing in recovery—even in the most difficult situations.
Limb salvage is not always the easiest path… but in some cases, it is the one worth taking.
